We want our community health centers to serve as your family’s medical home.

A patient-centered medical home is a model of care. This model coordinates patient care through a primary care physician. This coordination ensures the patient gets care when and where they need the care, in a way they understand. Greene County Health Care offers a variety of services to their patients. We also accept new patients and walk-in patients every day, at each of our clinics.
